On 5/27/26 at 3:22 PM Licensing Program Analyst (LPA), Mahjoba Mohsini, made an unannounced visit to deliver finding on the above referenced allegation. LPA met with the facility representatives Geneva Loken and Melinda Lopez. LPA toured the facility and observed 20 children with 7 staff as follows:
Room # 4A 8 Children with 2 staff (Karina Hernandez Castro and Shahnaz Ansari)
Room # 3; 3 children with 1 staff (Kristen Beauvais)
Playground; 9 children with 4 staff (Mitra Yazdanibiouki, Amanda Abrego, Jacquelyn Bulkowski and Steven Segarra)
The facility is within ratio and capacity.
During the investigation, LPA conducted observation, interviewes and reviewed relevant documents.
The information obtained shows that there were multiple child-to-child biting incidents. Staff reported the incidents occurred within seconds while shadowing and attending to a child. Staff attention was directed toward the child requiring additional support and adequate supervision of other children was not maintained, resulting in the biting incidents occurring. As a result, additional measures were implemented, including increased supervision, sensory and gross motor activities and added staff support during shadowing.

Type B deficiencies will be cited on the accompanying LIC 9099D.

Exit interview was conducted and report was reviewed with facility representative Melinda Lopez. Appeals Rights were reviewed and provided. Notice of Site visit posted during this visit and must remain posted for 30 days.

The facility representative, Melinda Lopez states facility has implemented additional measures to address supervision concerns and prevent future incidents, including increased supervision, sensory and gross motor activities and additional staff support while shadowing children requiring additional attention.

Based on interviews with staff, child-on-child bitings occured when staffs' attention was directed toward the child requiring additional support and adequate supervision of other children was not maintaine which is a potential health, safety and personal rights risk to children in care.

The facility will provide LPA documentation proof of the supervision plan and corrected measures to prevent future incidents and will also provide proof of plan to prevent any future incidents and will email to LPA by 6/30/26
