On 3/11/22 at 3:31 PM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management visit at facility’s request. The purpose of the visit was to approve and add classroom #18 to the school-age license. Upon arrival, LPA met with Site Director Elaine Gandy (Almario) and toured the facility. Census was 22 children with staff members Cynthia Bavaro and Natalia Espinosa in the licensed room #19. There were also 10 children in classroom #18 with SIte Director Elaine Gandy (Almario) that has not yet been approved by Licensing. LPA explained that using an additional room that is not licensed before approval is a violation of reporting requirements. Total census was 32 children which is over the current capacity of 30 and is in violation of the current fire code. Klassic Kids Program Manager Kristen Gonzales arrived around 3:20 PM.

Fire clearance was granted on 3/7/22 to add classroom #18 to current license approved for classroom #19 to serve children ages 4.9 through 14 years. Program manager changed license comments to serve children grades TK – 5th in Rooms #18 and #19. Capacity increase is requested for a total of 56 children.

During today’s inspection of the facility, in Room #18 at 3:35 PM, LPA observed two bottles of cleaning solutions and a bottle of disinfectant wipes accessible in unlocked cabinets. Program Manager Kristen Gonzales removed the items and put them up high on top of a cabinet out of reach of children in room #19. LPA observed furniture and equipment in good condition and free of sharp, loose or pointed parts. Director states there are no bodies of water or weapons on the premises. Medications are stored in a locked cabinet. All floors appear clean and safe. There are no signs of rodents or pests and food prep area is clean and free of litter. All storage containers for solid waste have tight-fitting covers that are in good repair. (continued on LIC809-C...)

Drinking water is readily available both indoors and out. Outdoor activity space has not changed. LPA observed playground equipment in good conditions and free of hazards. Bathrooms are clean and sanitary and allow for privacy. Facility has a working carbon monoxide detector. Staff and children’s records were not reviewed.
Behind the school there is a community park that is available to the public after school hours. The access is through a pedestrian gate that walks through the area where the licensed program operates. LPA took photos of area and will discuss with management to determine if additional fencing is needed.

Today, deficiencies cited under Title 22 Division 12 Appeal rights given.
Upon receipt of this report, licensee shall post and provide copies of this licensing report to parents /guardian of children in care at the facility and to parent/guardians of children newly enrolled at the facility during the next 12 months. Licensee to provide Acknowledgement of Receipt of Licensing Reports (LIC 9224) for each child in care and have each parent sign the form that they have received a copy of the report LIC 809 and LIC 809D.
THIS REPORT MUST BE FILED IN FACILITY FILE AND MADE AVAILABLE FOR PUBLIC REVIEW FOR 3 YEARS.

All proof of corrections (POCs) will need to be corrected before capacity increase and room addition can be granted. Exit interview conducted and report was reviewed with facility representative, Elaine Gandy and Program Manager Kristen Gonzales. A notice of site visit was given and must remain posted for 30 days.
