25 | On 2/21/23 at 2:30pm an office meeting was conducted with licensee representatives, Ed Stanfield, Association Director of Child Youth Developments, Lucelyna Godwin, Association Program Specialist and Marina West, Facility Director to discuss a self reported incident dated 12/7/22 and deficiency cited 12/14/22. Present at the meeting was Licensing Program Manager (LPM), Tulam Vu Licensing Program Analyst (LPA), Martha Malane.
On 12/14/22 during a case management inspection, the following deficiency was cited:
· Type A 101229(a)(1) Responsibility for Providing Care and Supervision – On 12/7/22 a child walked out of the outdoor activity space, through a classroom and to the courtyard without the supervision of a teacher.
On 11/20/19 a case management inspection was conducted due to a self reported incident where a child was left unattended in a restroom.
It is noted that the licensee corrected the above deficiencies in a timely manner. The director conducted a staff training to ensure children are supervised and provided the Department with a written plan of operation detailing how staff will ensure children are consistently supervised.
LPM discussed with licensee representatives the seriousness of the above deficiencies. Licensee representatives was informed that repeated violations or failure to comply with licensing laws and regulations may result in administrative action taken by the Department. Licensee representatives stated they understood and will ensure the facility complies with all regulations and laws governing Child Care Centers.
