25 | On 1/13/25 at 1:45 PM, Licensing Program Analysts (LPAs) Keturah Lane and Adriana Macias conducted an unannounced case management inspection to follow up on an unusual incident report received from the facility on 12/26/24 via fax. The facility called the duty line on 12/24/24 to report the incident that happened on that same day. Upon arrival, LPAs were greeted by staff member Tania Rodrigues and toured the facility. Director Maria Naranjo was on lunch break and arrived at approximately 2:00 PM. LPAs observed a total of 25 children in 3 classrooms with 5 staff members. All staff present were fingerprint cleared and associated to the facility.
During this visit, LPAs interviewed Director and inspected the area where the incident happened. The two staff members that witnessed the incident were out today and not available to interview. The child (C1) involved in the incident has been in attendance since 1/3/25, but left early today and was not available to interview. According to the Director, it was an unfortunate accident and the child trips a lot over own feet. Per Director, there is a climbing structure in the classroom and they put a mat out for the structure. While trying to get to the music area of the classroom, the child tripped over the feet while on the mat, and the forehead hit the middle shelf of the adjacent bookshelf. Both staff members were close by, but could not prevent the child from falling and hitting the forehead on the shelf. Director stated C1 did not put out hands to prevent the fall. The staff attending to the child provided first aid while the other called the mother to notify her of the injury. Medical treatment was needed and the child returned to the facility on 1/3/25. Director provided photos of the area (mat and shelf) as well as the cut on child's forehead. LPAs also talked with staff member S1 who also stated that child trips a lot both inside and outside the classroom. LPA Macias contacted the parent of C1 who stated concerns regarding the placement of the mat by the climbing structure. Director stated there have been no other injuries regarding the mat, climbing area or bookshelf where the incident happened.
No deficiencies were cited at this inspection. Exit interview conducted and report was reviewed with Director Maria Naranjo. Notice of site visit was provided and must remain posted for 30 days. |
