On 4/20/26 at 2:02 p.m. Licensing Program Analyst (LPA) Renita Rodriguez made an unannounced complaint visit for the complaint received on 1/14/26. The purpose of this visit is to continue the investigation and deliver findings on the above referenced allegations. LPA was granted entry after identifying self, showing badge, and disclosing the reason for the visit. LPA met with the Mentor Teacher Danielle Gameros. Upon arrival there were 8 staff and 48 children.

Regarding allegation "Staff yelled at a daycare child", LPA could not verify during interviews conducted, an incident occurred involving a child being yelled at. Interviews and review of staff files could not confirm or disprove the allegation and did not reveal any corroborating concerns. Per Director, staff may have a firm voice but do not yell at children. Per interviews staff has an understanding about personal rights of a child and what is not allowed. The evidence did not provide that staff witnessed or heard staff speaking to a child in an inappropriate manner.

Regarding allegation "Staff operated out of ratio", documentation for the month of May 2025 was reviewed. LPA did not find the center to be out of ratio based on sign in sign out documents reviewed for the staff and children. Per interviews the ratio followed is 1 staff with up to 10 children. LPA directly observed appropriate ratios on 3/23/26, 3/25/26, 4/10/26 and 4/20/26..

Regarding allegation "Staff left the daycare children unattended”, Director states she has not received information regarding children being unattended on the playground. LPA reviewed unusual incident reports for May 2025 and did not find evidence of a child left unattended for this time frame. LPA could not verify during interviews conducted, an incident involving children unattended on the playground. The sign in sign out sheets for staff and electronic sign in sign out documents for children found the center to be operating with appropriate counts for staff and children. Staff are provided with the option to reach out for support and request assistance as needed at any time for the care of the children.

Although the allegations may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violations occurred, therefore the above allegations are found to be Unsubstantiated. Exit interview conducted and report was reviewed with Danielle Gameros. A notice of site visit was given and must remain posted for 30 days. LPA observed form posted.
