On 6/20/21 at 10:45 AM, Licensing Program Analysts (LPAs) Keturah Lane and Selina Siao conducted an unannounced case management inspection. The purpose of the visit is to measure & request a waiver for temporary outdoor activity space while the current playground is under construction. Upon arrival LPAs met with Director Jennifer Marquez and toured the facility. Total census for the infant license today is 13 children. The following ratios were observed:

· Room #1 has 7 children with staff members Karina Haidar and Melinda Cumberbatch
· Room #2 has 6 children with staff members Allicia Jones, Sara Martinez and Victoria Villanueva

LPAs measured an indoor play warehouse area that will be temporarily used for outdoor activity space and an additional outdoor turf area while the new playground is under construction. Director stated that during construction the playground area will be barricaded and inaccessible to children. Director provided a schedule when the children will be using the indoor gross play warehouse and the turf area. The indoor play warehouse area measured 4,354.77 square feet which will accommodate no more than 58 children at one time. (continued on LIC809-C...)

Turf area located next to the Orange room measures 1,389.63 square feet, which will accommodate no more than 18 children. The total measurement for both areas is 5,744.40 square feet.

Existing infant playground was remeasured today and the total area was 3,656.66 square feet which will accommodate no more than 48 children.

No deficiencies were cited. Director provided updated LIC500, LIC610 and Emergency Response Plan Handbook.

An exit interview was conducted with the Licensee. The Licensee was provided a copy of their appeal rights (LIC 9058) along with the report (LIC809) and their signature on this form acknowledges receipt of these rights. The LIC 9213 (Notice of Site visit) was posted during today's visit. Notice of Site Visit must remain posted for 30 days.
