25 | On 10/12/21 at 9:15 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management inspection for the purpose of approving the new playground for use. Upon arrival, LPA Lane met with Director Jennifer Marquez and toured the facility. There were 3 children in the infant room with staff member Sarah Martinez. There were 8 children in the toddler room (they were outside in the corner zone at time of tour) with staff members Melinda Cumberbatch and Karina Haidar. Total census was 11 children. Director stated total enrollment at this time is 16.
LPA observed proper ratios and supervision. Staff members were wearing masks inside the classrooms. LPA Lane toured the playground. Playground is divided by a 5-foot chain-length fence into two areas for the preschool and infant licenses. Playground equipment is in safe condition, free of sharp, loose or pointed parts. Playground equipment is appropriate for ages 6-23 months. The surface of the outdoor activity space is free of hazards. There is adequate shade provided by large canopies. Director stated they take out a 5-gallon cooler of water and children can use disposable dixie cups when they wish to drink. LPA Lane approves the playground for use once the construction company removes the fences currently blocking entry for children.
An exit interview was conducted with Director. The DIrector was provided a copy of appeal rights (LIC9058) along with the report (LIC809) and signature on this form acknowledges receipt of these rights. The LIC9213 Notice of Site visit was observed being posted. Notice of Site Visit must remain posted for 30 days. |
