13 | On 12/29/2022 @ 10:45 a.m., Licensing Program Analyst, Joelle Redding, made an unannounced visit to initiate an investigation into the above-referenced allegations. During this investigation, LPA reviewed files and interviewed Director and staff and parents. Based on the information obtained, the facility did not report the power outage on 10/28/22, as an Unusual Incident, to Licensing as required by regulation. The power outage caused a potentially unsafe environment for the infants who require refrigeration for breast milk. With regard to the final allegation, there were at least two infants dropped off for care without the required immunization paperwork. Therefore, the preponderance of evidence standard has been met, and the above allegations are found to be SUBSTANTIATED. Type B deficiencies under California Code of Regulations, (Title 22, Division 12 & Chapter 1) are being cited on the attached LIC 9099D.
NOTICE OF SITE VISIT WAS GIVEN AND WILL REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
