13 | On 4/28/23 at 10:00am Licensing Program Analyst (LPA) Annette Sutherland conducted a complaint visit in order to deliver findings. Upon arrival LPA met with Director Carmen Polifrone. There were 13 children present and 4 staff members. The purpose of today's visit is to deliver findings regarding the above allegation.
Based upon staff and witness interviews, documents and photographs reviewed, the above allegation is substantiated. Investigation conducted found that on 3/8/23 facility failed to immediately notify parent when a child sustained injury more serious than a minor cut or scratch injury to the forehead. Parent was notified 3 hours after the incident. Although medical professional determined head injury did not seem serious at the time. Child had exhibited concussion symptoms which prompted the parent to seek medical attention. Medical professional advised parent to continue to watch symptoms of concussion. The injury the child sustained was more serious then a minor cut or scratch. Per regulation licensee should have notified parent immediately of injury.
The preponderance of evidence standard has been met, therefore the above allegation found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 or Health and Safety Code, are being cited on the attached LIC 9099D. Exit interview was conducted with Director Carmen Polifrone and a Notice of Site Visit was provided, posted and will remain posted for 30 days.
