Licensing Program Analyst (LPA), Jessica Rubio conducted an annual inspection as part of a compliance review. LPA met with Teachers Stephanie West and Christine Munsey. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:

· The following items were posted and updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Menu
· The facility is operating within the limits as stated on the license.
· Ratios are being met during this inspection. LPA observed 20 children in care with two teachers and an instructional aid providing care and supervision.
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards.
· LPA tested the carbon monoxide detector and it was not working properly. A citation will be issued. During the visit the battery was replaced and the carbon monoxide detector was tested again and working properly.
· There are no weapons present at the facility as stated by Principal Art Carrasco.
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Drinking water is provided in the indoor activity space by public water supply and in the outdoor activity space by filtered water supply.

· Medications are stored where inaccessible to children. When needed medications are stored in the health office.
· Hazardous items were not stored where inaccessible to children. LPA observed a storage cabinet with cleaning solutions that was not locked therefore making it accessible to children in care. Staff immediately moved some of the items and placed them higher level. A citation will be issued.
· All floors were observed to be clean and safe
· LPA observed ants in the classroom in the teacher's desk area as well as in the student bathroom. A citation will be issued.
· Bathrooms were observed to be safe, sanitary and in operating condition
· Playgrounds are enclosed by appropriate fences and free of hazards
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record were not reviewed as students are signed in electronically using The Learning Genie.
· Disaster drills are conducted at least every six months however documentation was not available for review.

A review of staff and children's records were conducted as part of this evaluation.
· 10 children’s records were reviewed. All children’s files were missing Consent for Emergency Medical Services (LIC 627), signed personal (LIC 613A) and parent rights (LIC 995) forms. Citations will be issued.
· Staff records review indicates that all staff present meet minimum qualifications for the position for which they were hired.
· A staff member is present with current Pediatric CPR/First Aid which expires on 4/1/2023
· Opening and closing staff member’s CPR/First Aid expires on 4/1/2023
· A review of staff records indicate that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following items were discussed with the Principal during inspection:


· Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall

· The Licensee was informed of their reporting requirements and provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov
· The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility is being cited for Title 22 Regulation Sections & Health & Safety Code. See LIC809-D for cited deficiencies.
