This is an amended version of the original report created on 12/27/2024.

On 12/27/2024, at 12:30 pm., Licensing Program Analysts (LPAs) Danielle Anderson and Michelle Hood conducted an unannounced case management inspection regarding a self - reported incident wherein a 3-year-old child was found unsupervised on the playground, while the children and teachers were inside the facility. LPAs met with Site Supervisor, Marissa Baca. LPAs discussed the purpose of the inspection and was led inside the facility. This report was created under the infant center in error and the incident occurred under the preschool license. The deficiencies cited from this report have been dismissed.

The following ratios were observed: Classroom 1 had 17 napping children present with 1 aide, Classroom 3 had 15 napping children present with one aide, and Classroom 2 & 6 were closed.

The site Supervisor was provided appeal rights (LIC 9058) and their signature on this form acknowledges


receipt of these rights. Notice of Site Visit (LIC 9213) was provided to be posted at the facility for 30 days.
LPA observed form LIC 9213 posted. An exit interview was conducted.

The Assistant Director is going to conduct a zoning and supervision training, staff will watch the Supervising Children in Child Care Centers on CCLD website, Active Supervision and Outside Play -

Based on documentation, video footage and interviews, the facility staff did not comply with the section cited above, as a child was left unattended on the playground for approximately 6 minutes before staff noticed the child on the playground. This poses an immediate health, safety, and personal rights risk to persons in care.

Site Supervisor will create a plan on how they will ensure a qualified teacher is present, at all times when children are in care. Site supervisor will conduct a meeting and provide LPA with a copy of the agenda which includes aides only working under the supervision of a teacher.

Based on video footage, staff interviews and site supervisor admintance 3 teachers aides were left alone with 7 children in care without a qualified teacher present.
