On 3/07/25, at 12:15 pm Licensing Program Analysts (LPA’s) Adriana Macias and Nancy Diaz, conducted an unannounced Case Management visit for the purpose of discussing a self reported incident that occurred on 2/20/25. The incident was self-reporter on 2/24/25. LPAs met with site director Samantha Aguirre and conducted a tour of facility. LPA’s observed the following ratios:

(Classroom Infant 2) - 9 infants, 3 teachers and 1 aid.

(Classroom Toddler 1) - 21 infants, 3 teachers.

(Classroom Infant 1) – 16 infants, 3 teachers and 1 aid.

(Classroom Toddler 2) – 7 infants, 1 teacher.

Incident reported involved an infant that was left on the floor by a teacher on top of a boppy pillow. A blanket was left above the infant’s head and due to lack of supervision, the infant was able to grab the blanket and dragged it over the infant’s head.

During this visit, LPAs interviewed Director and two staff members. Director stated that a training was conducted the next day about supervision and blankets not allowed in the classroom. Director also provided her plan of action and staff agreed to be more vigilant on the supervision of children. Part of their plan states that blankets or soft sheets are no longer allowed in the classroom.

LPAs Adriana Macias and Nancy Diaz informed facility representative Samantha Aguirre that this report dated 3/07/25 documents two Type A citations, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPAs Adriana Macias and Nancy Diaz informed the facility representative to provide a copy of this licensing report dated 3/07/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Pursuant to Title 22 of the CA Code of Regulations, the following Type A and Type B deficiencies were cited (refer to LIC 809-D).

Type A deficiencies if not corrected poses an immediate risk to the health, safety or personal rights of children in care.

Type B deficiency if not corrected poses a potential risk to the health, safety or personal rights of children in care.

Exit interview conducted and report was reviewed with facility representative Samantha Aguirre. A notice of site visit was provided and must remain posted for 30 days.

Based on staff interviews, and review of camera footage, it was observed that a parent walked into the classroom and found an infant's head covered with a blanket. Video footage reviewed also revealed that the infants head was covered for approximately 45 seconds.
