32 | It was alleged that staff did not put shoes on children in care, resulting in multiple falls. Facility Representative and staff members denied the allegation. Facility Representative and staff members stated that children always have shoes on when they indoors and during any outdoor play. Facility Representative and staff members stated that they are not aware of any children falling due to not wearing shoes.
Based on interviews conducted, there were no disclosures that corroborated the allegations. Due to conflicting information gathered throughout the investigation, the allegations are found to be unsubstantiated. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
A Notice of Site Visit (LIC 9213) was given to Facility Representative, Renece Mc Morris and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview conducted, and report was reviewed with Facility Representative, Renece Mc Morris.
