On 9/22/25, Licensing Program Analyst (LPA) Saraliz Velando conducted an unannounced complaint visit for the purpose of delivering findings for a complaint received on 7/9/25 regarding the above allegations. LPA met with the Director, Bailey Bowman and toured the facility. There were 4 staff and 10 infants present today. Appropriate ratios were observed. Staff members have the required background clearances and are associated with the facility.

LPA Velando conducted staff interviews, parent interviews, collected pertinent documents, and reviewed facility files. Based on the information obtained during the investigation, there was corroborating evidence that proved staff did not provide adequate supervision resulting in day care child sustaining injuries. A child sustained multiple injuries that required medical attention. During staff and parent interviews, it was also found that staff did not follow a parent's request as advised by child's doctor to alleviate a rash which resulted in that staff does not ensure day care child's diapering needs are met resulting in recurrent diaper rashes.The preponderance of the evidence has been met and therefore, the above allegations are found to be SUBSTANTIATED. Type B violations were cited. Refer to the next page LIC 809-D for deficiency citations. The exit interview was conducted with Director, Bailey Bowman. Appeal Rights and a copy of the licensing report were provided. A notice of site visit was posted and must remain for 30 days.
