25 | On 3/04/2025 at 8:47 AM, Licensing Program Analysts (LPAs) Adriana Macias and Nancy Diaz conducted an unannounced case management inspection to follow up on a self-reported unusual incident. Incident took place 2/21/25 at approximately 3:53 PM and Incident Report was submitted to licensing on 2/28/25. Upon arrival, LPAs met with Director Ana Lopez and toured the facility. LPA Nancy Diaz also delivered an amended report (dated December 5, 2024). LPAs observed the following census at the facility:
Classroom Monkeys: 17 children, 1 teacher, 1 aide
Classroom Bumblebees: 7 children, 1 teacher
Classroom Butterflies: 16 children, 2 teachers
Classroom Ladybugs: 21 children, 2 teachers
There was a total census of 61 children today. LPA observed Monkeys Classroom out of ratio with 17 preschool children, only 1 fully qualified teacher and an aide. LPAs observed a second qualified teacher come inside the classroom about 2 minutes later because she had gone to her break.
Incident reported involved a child left unattended for approximately 6 minutes on the playground. At this visit, LPAs interviewed Director and staff who found the child. LPAs observed the area where the incident had occurred. Due to staff lack of supervision, it was determined that the child was accidentally left in a licensed area of the playground and was not in immediate danger. Teacher in charge of that classroom, had failed to do a “Name to Face” count during the transition from playground to the classroom and did not realized that a child had been left behind inside one of the playhouse structures. Approximately 6 minutes after, the next classroom using the playground came outside and a couple teachers found the child inside the playhouse structure. Staff reported that child was observed playing and not in distress. Director stated she retrained staff on “Name to Face” census counts and provided Training Agenda.
Pursuant to Title 22 of the CA Code of Regulations, the following Type B deficiencies were cited (refer to LIC 809-Ds). Exit interview conducted and report was reviewed with facility representative Director Ana Lopez. LPAs provided a copy of the report and appeal rights. A notice of site visit was provided and must remain posted for 30 days.
