On 04/22/2026 at 2:15pm, Licensing Program Analysts (LPAs) Adriana Macias and Qwatevyia Edwards conducted an unannounced initial complaint inspection regarding the above allegations. LPAs met with Director Ana Lopez and Assistant Director Maria Cabrera and discussed the reason for the visit. The following census was observed: 96 children and 5 staff in 5 Classrooms during nap time. All staff present were fingerprint cleared and associated to the facility. Appropriate ratios and capacity were observed. During inspection LPAs requested a facility roster, personnel report and staff were interviewed.

Based on the documentation reviewed and interviews conducted, it was determined that a child’s personal rights were violated when a staff member recorded and posted the child in an inappropriate manner and without permission. In a separate incident, a child was left without proper clothing, again violating the child’s personal rights, and the authorized representatives were not notified, resulting in the child remaining without appropriate clothing from approximately 11:15 a.m. to 5:20 p.m. (CONT ON LIC9099C)

LPA Adriana Macias informed licensee Director Ana Lopez that this report dated 4/23/2026 document 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Adriana Macias informed Director Ana Lopez to provide a copy of this licensing report dated 4/23/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to parents/guardians of any newly enrolled child for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

The allegation is valid because the preponderance of the evidence has been met, therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12, Chapter 1) the deficiencies are being cited on the attached LIC 9099D. Exit interview conducted and report was reviewed with Director Ana Lopez. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were provided and discussed.
