On 09/17/24 at 11:20 am Licensing Program Analyst (LPAs) Gerald Poindexter and Mahjoba Raofi were conducting another matter at the facility. LPAs met with Gloria Hughes, director and Emilie Cranford, assistant director. LPAs discussed the regulation related to that other matter (and the resulting Type B citation) and the allegation that “Staff did not provide adequate supervision resulting in day care child sustaining an injury.” When staff became aware that the child’s injury required medical attention, the facility did not report the incident to the Department, as required. Additionally, based on LPA interviews with staff, the toy object in the classroom that was involved with that incident, was not age appropriate.

Exit interview conducted and report was reviewed with Gloria Hughes and Emilie Cranford. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were provided.

Director Gloria Hughes stated she will a submit UIR regarding the 10/17/23 incident along with a written statement that she understands and will abide by reporting requirements for future incidents. Director will email both documents to: Gerald.Poindexter@dss.ca.gov by 9/30/24.

Based upon facility documents, CCLD duty call records and staff interviews, an unusual incident report was not reported according to requirements above which is a potential health, safety and personal rights risk to children in care.

Following the injury incident involving the toy, Director Gloria Hughes acknowledged that the toy was not age appropriate and removed it from the classroom. This violation has been cleared.

Based upon staff interviews and video evidence, C1 was injured by another child using a toy fishing rod and bait that was not age-appropriate for the classroom, which posed a potential health, safety and personal rights risk to children in care.
