25 | On 3/25/2024, at 2:15 p.m., Licensing Program Analyst (LPA) Cindy Meier conducted an unannounced Case Management visit for the purpose of a follow-up on an Unusual Incident. LPA met with Site Supervisor, Shanyce Samuelu. LPA discussed the purpose of the visit and was led on a tour of the facility. There were sixty-four (64) children, and six (6) staff present at the time of this inspection.
On 1/26/2024, the San Diego Regional Office received an Unusual Incident Report which stated child (C1) told her mother that on 1/24/2024 child (C2), child (C3), and child (C4) touched her inappropriately during both class time and Character Builders time. Associate Executive Director (S1) discussed the situation with McKinley Principal (S2). S1 conducted an internal investigation with Character Builders staff (S3) and (S4) and filed a CPS report. After receiving this information, the Character Builders staff implemented additional supervision and ensured C1 was never out of visual sight of staff. No other incidents occurred.
During the course of the visit, LPA conducted interviews with S3, S4, S5 and C1 and reviewed documents related to the incident. Character Builder staff interviewed denied seeing any inappropriate touching by C2, C3, or C4.
Based on witness interviews conducted, internal investigation, and unusual incident report submitted, it was determined that on 1/24/24, S1, S3, S4 acted in a manner consistent with regulations and intervened to protect the personal rights of C1.

An exit interview was conducted with Site Supervisor, Shanyce Samuelu. A Notice of Site Visit is provided and required to be posted for 30 days.
