On July 11th, 2022, 2022 at 7:50 AM, Licensing Program Analyst (LPA) Jo Ann Legaspi conducted a case management inspection to follow-up on an incident that occurred on 06/13/2022. LPA advised Director Jimenez of the meeting’s purpose and was granted facility entry.

The facility serves children ages 2 – 5 years. The following ratios were observed in 4 classrooms: Room 1 had thirteen (13) children (aged 2 - 3 years) supervised by two (2) teachers. Room 2 had sixteen (16) children (aged 3 - 4 years) supervised by two (2) teachers. Room 3 had twelve (12) children (age 4 - 5 years) supervised by two (2) teachers. Room 4 had seven (7) children supervised by two (2) teachers. Appropriate ratios were maintained, and visual observation noted. Staff present has required criminal background clearances.

On 06/15/2022, the facility self reported an incident which occurred on 06/13/2022. Child 1 stated that on 06/13/2022, Staff 1 grabbed C1's cheeks and scratched C1 under their chin. (See LIC 811 Confidential Names). The investigation involved interviews with staff, parent and children. C1's parent stated their child stated S1 grabbed C1's cheeks and accidentally scratched under the child's chin. S1 denied they grabbed C1's cheeks and scratched them under their chin. C1 told LPA that nobody grabbed their checks and their dog scratched their neck. Other present staff members and C1's classmates reported C1 was not alone with S1. Staff members and other children denied observing any staff grabbed any child's cheeks and scratching under the child's chin. Per the director, there are no cameras in the classroom. Due to conflicting statements and obtained information, the allegation staff violated a child's personal rights has been determined to be unsubstantiated.



A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Director Cristina Jimenez.
