On 10/04/2022 at 10:30am, Licensing Program Analyst (LPA) Selina Siao conducted an unannounced inspection to investigate the above allegations. Upon arrival, LPA Siao met with Director Cristy Novoa and conducted a tour of the two toddler classrooms and the preschool classrooms. During the tour LPA observed teacher aides Zahra Ahmadkhah and Shahrbanoo Mousavi at the toddler playground supervising 7 toddlers as teacher Ledy Rodriguez Salgado had step away to the restroom.
During today's inspection, LPA inspected the toddler playground and the sand areas around and under the climbing structure and the two slides are compacted and does not have sufficient cushioned to absorbs falls. Based on LPA’s observations and interviews which were conducted and record reviews the preponderance of evidence standard has been meet regarding the above allegations, therefore the above allegations are found to be SUBSTANTIATED, California Code of Regulations, 101216.2(e) and 101238.2(e)(1) are being cited on the attached LIC 9099D.
Appeal Rights were discussed and provided. Notice of Site Visit was posted during this visit and will remain posted for 30 days.

Ahmadkhah and Shahrbanoo Mousavi were supervising 7 toddlers at the toddler playground while the toddler teacher Ledy Rodriguez went to the restroom. This poses a potential health and safety risk to clients in care.
