25 | On 06/08/22 at 10:00AM, LPAs Luigi Gargaro and Cindy Meier conducted an unannounced case management visit to the facility to deliver a citation for an out of ratio violation. During a complaint investigation visit, it was determined that the facility was operating out of ratio during the window of the complaint period, 03/14/22-03/25/22, in both its green and blue classrooms. Analyst determined that both classrooms were operating with just one qualified teacher and two or more aides that have insufficient ECE units and allowed for the supervision of fifteen children in each classroom. This occurred for five days in the green classroom and for eight days in the blue classroom during that period. Both classrooms were over 15 children for the days in question and were out of ratio during a subsequent complaint inspection visit on 04/06/22.
Facility was cited a type A deficiency on the following 809D page for violation. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.
The deficiency was corrected with the 05/17/22 opening of the third purple classroom with a qualified lead teacher and aide. Analysts inspected the classrooms today and found the following ratios: Green Room had 13 children and 2 staff with teacher Kelsey Oaks and aide Christalee Hill, Blue Room had 14 children and 2 staff with teacher Rosie Prieto and aide Ericka Melendez and Purple Room had 12 children with teacher Nadia Aldana and aide Gabriela Benevides.
An exit interview was conducted with supervisor Patty Salazar. A copy of this report, along with Appeal Rights (LIC9058 01/16), were provided. A notice of site visit was given and must remain posted for 30 days. LPAs observed that the notice of site visit was posted during the visit. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
