32 | This facility provides Incidental Medical Services – IMS. The director states that there are currently no children enrolled who require IMS services. Emergency information and medical assessment forms were reviewed for some children. The director states that admission agreements are not maintained for the children enrolled. LPA reviewed the sign in/sign out sheets. Five of the 30 children present were not signed in by a parent/guardian. Staff records contain documentation of education, training, and/or experience. The director states that the facility's water has not been tested for lead as required. Children are provided filtered water to drink. Menus are posted. LPA obtained an updated Emergency Disaster Plan for Child Care Centers- LIC610, Personnel Report-LIC500 and Children's Roster-LIC9040.

Director Andrade was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
A notice of site visit was given and must remain posted for 30 days.
