On 11/15/21 at 1:30 PM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced complaint inspection for the purpose of delivering findings regarding the above allegations. LPA Lane met with staff member Therese Riffey and toured the facility. Total census was: 13 children in the nap room. LPA observed children sleeping on mats with staff member Susanna Grossman present.
The Department fully investigated the above allegations and obtained information from facility file review, photos, facility documents as well as from interviews with complainant, enrolled children, parents/guardians of enrolled & previously enrolled children, staff members and the Director. It was found that although the allegations may have happened or are valid, there was not a preponderance of evidence to prove that staff slapped a child, yelled at the children or did not properly supervise a child in care. Therefore the allegations are UNSUBSTANTIATED. An exit interview was conducted with staff member Therese Riffey. Notice of Site Visit (LIC9213) was given and must remain posted for 30 days.

LPA Lane met with staff member Therese Riffey and toured the facility. Total census was: 13 children in the nap room with staff member Susanna Grossman.
The Department fully investigated the above allegations and obtained information from facility file review, photos, facility documents as well as from interviews with complainant, enrolled children, parents/guardians of enrolled & previously enrolled children, staff members and the Director. During the investigation, LPA Lane observed two potential hazard areas in the "big playground area". The allegation is valid because the preponderance of the evidence has been met and is therefore SUBSTANTIATED. See LIC9099D for deficiency cited. An exit interview was conducted with staff member Therese Riffey. Notice of Site Visit (LIC9213) was given and must remain posted for 30 days.

Based upon LPA observation, the fence has an area that has sharp metal & accessible to children as well as a gap between the wall and fence where a child could squeeze by. The other side of the playground not in use is not fully enclosed by a fence and potenetially accessible to children. This is a potential health, safety and personal rights risk to children in care.
