25 | On 11/30/2023 @ 1:50PM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced case management in reference to a self-reported unusual incident that occurred on 10/09/2023, 10/10/2023 & 10/11/2023. The unusual incident report indicated that there were 4 children (ages 8 years old) were involved in inappropriate activities under a blanket. One of the children reported this incident to the director on 11/8/2023.
Observed present today were 17 school-age children and staff Megan Waples & Samantha Seabrooks in Room B13.
Ms. Albertson stated that there were 8 children on 10/09/2023; 9 children on 10/10/2023 and 11 children on 10/11/2023 with one teacher. Staff who was in charge of supervising these children did not observed the inappropriate activities that happened on 10/09/2023, 10/10/2023 & 10/11/2023. Ms. Albertson stated that the staff is no longer employed at KCE. Ms. Albertson stated that on November 11, 2023 during a professional development day, staff were trained on active supervision.
Type A deficiency was cited today.
LPA Nancy Diaz informed site director, Victoria Albertson that this report dated November 30, 2023 documents a Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Nancy Diaz informed the director, Victoria Albertson to provide a copy of this licensing report dated 11/30/2023 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. |
