On 09/29/2023 at 11:01am, Licensing Program Analysts (LPAs) Selina Siao and Nancy Diaz conducted an unannounced inspection to initiate an investigation, for the complaint received on 09/19/2023 regarding the above allegations. Upon arrival, LPAs met with Director Jenni Gonzalez and conducted a tour of the classrooms to gathered census. Appropriate ratios were observed in both of the infant rooms. Throughout the course of investigation, interviews were conducted with several staff members and several day care parents. Based on the information obtained during interviews, facility do not return the zippy cups to the parents at the end of the day in the older infant room. There has been recent incidents within the last few weeks that children sustained bite mark and bruises while in attendance and staff failed to inform parents of the injuries during pick up. Facility also serves raisins and craisins to the older infants. The above allegations are valid because the preponderance of the evidence have been met, therefore, the above allegations are found to be substantiated. See attached LIC 9099D for type B citations issued. The Notice of Site Visit was provided and it must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representatives.

In the case of less serious injuries including, but not limited to, minor cuts, scratches and bites from other children requiring assessment and/or administration of first aid by staff, the licensee shall document the injury in the child's record and notify the child's authorized representative of the

Director stated that they will provide training with the staff members that any injury to a child, no matter how small including scratch, small bite or mark needs to be documented and notify the parent.

This requirement is not met as evidence by: Per interviews conducted there have been incidents where a child sustained a bite and injury and the authorized representative is not informed of the nature of the incident during pick up. This poses a potential health and safety risk to children in care.
