On 2/17/2023 at 10:00am, Licensing Program Analyst (LPA) Selina Siao conduct an annual inspection. LPA proceeded to the Director's office but was told by staff Raquel Ramos who was at the playground that the Director is in the infant room #3. LPA proceed to the infant room to inform the Director of the purpose of LPA's inspection. The following ratios were observed: Room 5 (Foxes) had 9 children at the playground supervised by teacher Jill Ricketts aka Tripes. Room 2 (Racoons) had 15 children supervised by teacher Nadiuska Castro and teacher aide Elizabeth Torres. Room 1 (Owel) had 7 children supervised by teacher Wafaa Sfeer. Facility is within staffing ratio. All required notices, forms and license were posted in the office. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water from an operational water fountain. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of mats which are kept in the classroom and each mat has its own sheet or covering. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels are not available for the children in the restroom located outside the playground. Toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for meals and snacks. Cleaning supplies are kept out of reach of children. Outdoor play area is a fenced playground with sufficient rubber chips for cushioning. Climbing structures, swings and slides are securely fixed to the ground. Area has a tree and shade structure used for shade. Equipment is age appropriate and separated by age groups. Area has an operational drinking fountain and grounds are free of debris or potential hazards. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage. Isolation area is the Director's office. Personnel and client records were reviewed. Not all staff members have

the required immunization record available for review or the online mandated child abuse training certificates. All personnel have required criminal record and child abuse index clearances or exemptions. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 11/15/2023. Facility has an operating carbon monoxide detector near the kitchen. Facility is in the process to get all the opening and closing staff members a valid EMSA approved CPR and First Aid cards since 1/23/2023.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
