On 6/27/25 at 8:30am Licensing Program Analysts (LPA) Mahjoba Mohsini conducted an unannounced visit to deliver findings regarding the above allegation. Upon arrival, LPA's met with Center Director, Elizabeth Hann.
LPA observed there were 41 children and 7 teachers (3 fully qualified and 4 aides). Appropriate ratios were met.

Based on interviews with staff and parents, LPA confirmed incidents where children’s diapers were not changed promptly. Staff offered varying explanations for the delays therefore the allegation is substantiated, meaning a preponderance of the evidence supports that the violation occurred.

Deficiency was cited. See LIC 9099D.
An exit interview was conducted with Director Elizabeth Hann. The report was reviewed and provided. Appeal Rights were discussed and issued. A Notice of Site Visit was provided and must remain posted for 30 days.

Assistant director provided documentation to LPA on 6/27/2025 that the facility has implemented a written diaper log as additional step to ensure staff are regularly checking and changing diapers. This deficiency is cleared.

Based upon staff interviews, at least 1 child was left in soiled clothing for extended period of time which posed a potential health, safety and personal rights risk to children in care.
