On 10/1/21 at 9:35 AM, Licensing Program Analyst (LPA) Keturah Lane visited the facility to conduct an annual inspection. Upon arrival LPA met with Director Izhar Mohamed and proceeded to tour the facility. Owner Anab Hade arrived at 10:00 AM. The facility re-opened on September 1, 2021 but no children are enrolled at this time. Director and owner are currently advertising for enrollments.

Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Director stated that she will be purchasing water bottles for the children (once enrolled) so drinking water will be readily accessible. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Director stated once children are enrolled that adequate food will be available for meals. Cleaning supplies are kept separate from food and are inaccessible to children. Storage containers for solid waste have tight-fitting covers and are kept in good repair. Director stated there were no poisons on the premises. Director stated there are no firearms or other weapons on the premises. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 F or less. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility. This license has a toddler component. Facility has a playground waiver that allows children to use the play area at alternate times.

The toddler outdoor play area (patio) is fenced and completely covered. There are no large play structures. Director stated that balls and portable play equipment is stored in a locked storage area and is brought out daily. The preschool outdoor play area is a short walk down the strip mall. This area is completely fenced and the surface is concrete. There are no play structures. Director stated that each morning the area is cleaned of any trash, debris or potential hazards before children use the area. (Continued on LIC809-C...)

Portable play equipment is brought to the play area. There is a wooden structure that provides shade on one side of the play area. There are no bodies of water or weapons at this facility. Equipment is age appropriate. Director and owner will be purchasing additional play equipment once children are enrolled.

LPA reviewed area for medication storage. At this time since there are no enrollments so there is no medication in storage. Director stated that they only accept prescription medications as necessary. Director Izhar Mohamed is a new fully qualified preschool director. At this time personnel is Director Izhar Mohamed and Owner Anab Hade. Director is currently interviewing potential staff members. Personnel records (Director and Owner) contain documentation of education and at least one staff member has current CPR and First Aid certifications. All current staff have completed mandated reporter training and required immunizations. Sign ins were not reviewed. Children’s records were not reviewed.. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions.

No deficiencies were cited.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies. An exit interview was conducted with the Director. The Director was provided a copy of their appeal rights (LIC 9058) along with the report (LIC809) and their signature on this form acknowledges receipt of these rights. The LIC 9213 (Notice of Site visit) was posted during today's visit. Notice of Site Visit must remain posted for 30 days.
