On 11/21/24 at 9:00 am, Licensing Program Analyst (LPA), Gerald Poindexter, met with Anab Hade, facility owner, for the purpose of an unannounced annual inspection. Upon LPA arrival Ms. Hade was in her car driving away from the facility when she noticed LPA Poindexter. She parked her car and returned to the facility. LPA Poindexter then identified himself and the purpose for the visit. Also present at the facility was office assistant Fardowso Mohamud (who provided translation for Ms. Hade from Somali to English), Sukhri Abdi an teaching aide, and Fahan Hirsi the licensee’s non-staff, adult daughter. Ms. Hade identified Sahra Mohamed as the center director, but stated that she was in Minnesota “until December.” Ms. Hade stated that she attempted to reach the director by phone with no response. There were 7 children present, ranging in age from 17 months to 4 years) with three staff in one room.

LPA toured the facility. The rooms were clean, orderly and a comfortable temperature during this visit. Adequate ventilation and heating are available. The books, games, and toys are safe, age-appropriate and in good repair. There are a variety of activities available throughout the day. All required forms were posted. All storage containers and trashes containing solid waste have tight fitting lids and are in good repair. There is kitchen which is clean and sanitary. Food is stored in covered containers at 45 degrees or less and there is no expired or contaminated food present. Staff preparing food are using proper personal hygiene and food service practices. The food meets the nutritional requirements per regulation and is of good quality and proper quantity. The lunch/snack menu is posted, changes are recorded, and menus are stored for 30 days. All food has not been stored separately from any chemicals or cleaning products. Drinking water is readily available. Napping equipment is sufficient for each child, bedding is stored separately, and mats/cots are disinfected after use. Facility has ensured that there is adequate space between mats/cots for easy passage and they are not blocking entrances or exits.

Hand washing and toileting areas are in a safe, sanitary, and operating condition. Any wastewater used to clean is being discarded after use. Medications are kept in the director’s office, inaccessible to children. Poisons, disinfectants, cleaning solutions and other items that are dangerous to children were not made inaccessible. LPA observed construction materials labeled “taping & topping joint compound” in the patio used for toddler play. There is no evidence of rodent or insect activity. Pre-school outdoor play area is fully fenced with and adequate shade. Ms. Mohamud stated that the area has not been in use for several months due to homeless activity. Age-appropriate playground equipment and outdoor surfaces have not been maintained while the playground was not in use. LPA reminded licensee that preschool and toddler playgrounds cannot be shared, in accordance with the waiver provided by the department. There are no bodies of water, firearms, or ammunition on the property. The carbon monoxide detector is operational. The facility has a written disaster plan in place that meets regulatory requirement and has been conducting and documenting evacuation drills every six months. The facility transports children.

LPA reviewed sign in/out sheets, a sample of personnel records and a sample of children's records. Children’s records were incomplete. Staff records were not made available to LPA at the time of inspection. License stated that the director keeps them locked and that she did not have access. Staff present did not have current CPR and First Aid certification. Facility is reminded the Mandated Reporter Training is to be retaken every two years and can be accessed at the following website: www.mandatedreporterca.com.



Children are evaluated upon entry and monitored throughout the day for signs of illness. The isolation area for ill children awaiting pick up is the director’s office Reporting requirements for positive Covid-19 results in children or staff were discussed to include contact with County Department of Public Health for guidance (619-692-8499) and Licensing (619-767-2248) to report the unusual incident for three or more cases.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the licensee, confirmed that there are no Registered Sex Offenders in the facility and LPA completed the RSO profile in FAS.

LPA Gerald Poindexter informed licensee Anab Hade that this report dated 11/21/24 document(s) two Type A citations which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Poindexter informed the licensee Anab Hade to provide a copy of this licensing report dated 11/21/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Please be advised that FAILURE TO PAY the required civil penalty payment may result in the REVOCATION OF YOUR LICENSE. You must respond within 30 days with the payment of or a proposed payment plan that includes the first payment. Further, the Department will not approve any requests for increase in capacity or for additional capacity of additional licenses while civil penalties remain unpaid.

LPA conducted child care quality management interview with the licensee Anab Hade. Exit interview conducted and report was reviewed with the licensee Anab Hade. LPA discussed in exit interview with the licensee that the facility cannot reopen to child care until the licensee can provide proof of criminal background clearance for all staff. Licensee must also provide personnel records as proof of fully qualified staff before the facility can resume caring for children. During the inspection, all children were pick up and removed from the facility by parents. NOTICE OF SITE VISIT WAS GIVEN AND WILL REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights provided.
