On 11/8/23 at 1:15 PM, Licensing Program Analyst (LPA) Adrian Mangina visited the facility to conduct an annual inspection. Upon arrival LPAs met with Director Michelle Kira. Licensee Paige Kratz was also present. LPA toured the facility with Director Kira. and proceeded to tour the facility. During today's inspection, there were 31 children with four staff in 3 classrooms. Appropriate ratios, capacity and supervision were observed.

Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Drinking water is readily accessible. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Licensee advised to ensure all menus are current. Adequate food is available for snacks. Children bring their own lunch, and snacks are provided by the facility. Cleaning supplies are kept separate from food and are inaccessible to children. Storage containers for solid waste have tight-fitting covers and are kept in good repair. Storage areas for poisons are locked. Director stated there are no firearms or other weapons on the premises. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 F or less. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility. Last fire drill was conducted on 11/8/23 and last earthquake drill was conducted on 10/19/23. All required postings were present.

Outdoor play area is a fenced playground with sufficient material for cushioning. LPA observed two full water tables on playground where children were present unattended and accessible. Licensee states there are no weapons at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has

Based on observation, the licensee did not comply with the section cited above as two water tables were left filled and unattended while children were present and playing in the playground which poses an immediate health, safety or personal rights risk to persons in care.

trees and large canopies used for shade. Equipment is age appropriate. Area has drinking water readily accessible and grounds are free of debris or potential hazards.

LPA reviewed medication storage. LPAs observed 1 child’s medication (inhaler)that was properly stored with the original label in the office, with dosage properly documented. LPA reviewed personnel records and at least one staff member has current CPR and First Aid certifications. Licensee was advised to ensure all staff have current mandated reporter training certificates. All staff have required immunizations. Each personnel record contains documentation of educational background and training. Sign ins were reviewed. Children’s records contain admission agreements and medical assessment. All facility staff who require caregiver background checks have received criminal record and child abuse clearances or exemptions.

Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

LPA Mangina informed licensee Paige Kratz that this report dated 11/8/23 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Mangina informed the licensee to provide a copy of this licensing report dated 11/8/23 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with the Facility Representative Michele Kira. A notice of site visit was given and must remain posted for 30 days.
