On 7/26/22 at 8:50 AM Licensing Program Analysts (LPAs) Adrian Mangina and Keturah Lane made an unannounced complaint visit for the complaint received on 5/24/22, regarding the above allegation for the purpose of reviewing staff files, obtaining sign in and sign out sheets and to deliver findings on the above reference allegation. LPAs met with Executive Director, Shelly Baker. Also present were (5) daycare children and four teachers. Proper ratios and supervision were observed. LPAs reviewed staff files and obtained sign in sign out sheets.

It was alleged that Director is not on site during business hours. Based on the information obtained during interviews, observations, and documentation reviewed it is determined that the facility Director and Assistant Director left the facility abruptly at the end of March or early April. Excetutive Director Shelly Baker covered the Director position until a new qualified Director was hired 6/2/22. Facility Representative was unable to provide proof that this change was reported to Licensing within 10 days as required.

The allegation is valid because the preponderance of the evidence has been met, therefore, the above allegation is found to be SUBSTANTIATED. Exit interview conducted and report was reviewed with Facility Representative Shelly Baker. The Licensee was provided a copy of this report. A Notice of Site Visit (LIC9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

On 7/26/22 at 8:50 AM Licensing Program Analysts (LPAs) Adrian Mangina and Keturah Lane made an unannounced complaint visit for the complaint received on 5/24/22, regarding the above allegations for the purpose of reviewing staff files, obtaining sign in and sign out sheets and to deliver findings on the above reference allegation. LPAs met with Executive Director, Shelly Baker. Also present were (5) daycare children and four teachers. Proper ratios and supervision were observed.

It was alleged that napping equipment is not being properly arranged and lack of supervision resulting in child being injured. on 6/24/22, he facility underwent a change in enrollment and moved the two existing preschool classes to another affiliated facility nearby and LPA was unable to observe the napping layout of the chldren while children were present, however, interviews indicated that sufficient space (2 feet) was left between napping children. Based on the information obtained during interviews, observations, and

and documentation reviewed it is determined that the allegations are UNSUBSTANTIATED. Although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred, therefore the above allegations is found to be Unsubstantiated.

Exit interview conducted and report was reviewed with Facility Representative Shelly Baker. The Facility Representative was provided a copy of this report.
