On 08/17/2021, at 2:16 p.m., Licensing Program Analysts (LPA), Casey Gulley conducted an unannounced case management inspection with Julia Dukes and Peggy Parameshwaran. 38 children with 12 staff present in the facility during this inspection.

During a unannounced inspection, it was observed and reviewed by LPA staff member #1(S1), was not fingerprint cleared and associated to the facility until June 30, 2021. S1 was employed with Kinder Mission Day Care Center from November 2020 until July 2021. Julia Dukes admitted S1 was employed at the facility and S1 was terminated as of July 13, 2021.

AB633 requires upon receipt, the Licensee shall post (observed by LPA) and provide copies of this licensing report to parents/guardians of children in care at the facility and parents/guardians of children newly enrolled at the facility during the next 12 months. An Acknowledgment of Receipt of Licensing Reports, Form LIC 9224 must be signed and placed in each child’s file.

A copy of this report, LIC 809D, appeal rights (LIC 9058), LIC 9224, and LIC 9213 – Notice of Site Visit were provided to the licensee. LPA observed licensee posed the LIC 9213 and the licensee was advised to post the LIC 9213 for 30 days. An exit interview was conducted with the licensee.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, 809 D) An exit interview was conducted with Julia Dukes and Peggy Parameshwaran. Licensee and Director provided a copy of their appeal rights (LIC 9058)

Director stated that she will verify with LPA prior to staff member being present in the classroom. Also, Director will verify staff members eligibility monthly by contacting and verifying with assigned LPA.
