On August 30, 2021 at 4:00 p.m. , Licensing Program Analyst (LPA) Casey Gulley conducted an unannounced Case Management inspection. LPA met with Julia Duke, to deliver an amended licensing report originally created on 8/17/21. LPA was granted entry into the facility by Julia Duke . (18) eighteen children and (5) five staff were present in the facility during this inspection

An exit interview was conducted. LPA observed Julia Duke placing the Notice of Site Visit on the wall visible to parents during today’s inspection.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. An exit interview was conducted with Julia Duke. Julia Duke was provided a copy of their appeal rights (LIC 9058).
