On 05/11/22 at 12:00 p.m., Licensing Program Analyst (LPA), Casey Gulley, conducted an unannounced Case Management inspection in order to deliver an amended report originally created on 5/10/2022. Upon arrival, LPA met with Director Julia Duke and proceeded to tour the facility. During the inspection, there were sixty- five children in care with fourteen staff members present. No deficiencies cited.

Exit interview conducted and report was reviewed with Director Julia Duke. A copy of this report, along with Appeal Rights (LIC9058 01/16), were provided. A notice of site visit was given and must remain posted for 30 days. LPA observed that the notice of site visit was posted during the inspection. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
