On May 28, 2025, at 11:30 AM, Licensing Program Analyst (LPA), Diana Sanchez made an unannounced complaint inspection to deliver the complaint investigation findings for the above allegations. LPA met with director Kristin Mahaffey and explained the purpose of today’s inspection. Current census is 30.

This agency has investigated the above listed allegations. During the investigation, LPA conducted facility inspections, interviews with facility staff, daycare children and review of children’s records.

It was alleged that on November 7, 2023, daycare child #1 (C1) sustained an injury while in care and facility staff did not notify authorized representative of C1’s injury in a timely manner. Facility director denied the allegation, stating that although C1 sustained an injury, the incident was reported to C1’s parent (P1) and a copy of the ouch report was signed by P1 upon pick up.

It was alleged that staff did not prevent C1 from being bullied while in care. Facility staff denied the allegation stating that they never saw other children picking on C1. During interview with LPA, C1 disclosed that some daycare children were mean, but could not provide any additional details or names of children involved.

It was also alleged that on September 3, 2024, staff did not prevent inappropriate interaction between C1 and daycare child #2 (C2). A facility file review indicates that the alleged incident was reported to the San Diego Child Care Regional Office (SDCCRO) on September 9, 2024. SDCCRO concluded full investigation into the incident on September 26, 2024, including review of video footage and interviews of all pertinent parties. Both children and staff denied any inappropriate interaction or absent staff supervision. During the course of the current investigation, no additional evidence was obtained to support the allegation.

Due to inconsistent testimony, lack of supporting evidence and no other witnesses to the alleged incidents, LPA was unable to determine whether or not the above allegations occurred. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

Exit interview was conducted and this report was reviewed with director Kristin Mahaffey. A Notice of Site Visit was provided and must remain posted for 30 days. LPA observed that the notice was posted during the inspection. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
