On July 2, 2025, at 11:30 AM, Licensing Program Analyst (LPA), Diana Sanchez made an unannounced complaint inspection to deliver the complaint investigation findings for the above allegations. LPA met with facility owner Lynn Twork and explained the purpose of today’s inspection. Current census is 23.

This agency has investigated the above listed allegations. During the investigation, LPA conducted facility inspections, interviews with facility staff, daycare children and reviewed of children’s records.

It was alleged that on May 23, 2024, daycare child #1 (C1) sustained a wood splinter in their knee and facility staff did not assess or administer first aid to C1’s injury. Facility staff denied the allegation stating that they always assist children when there is an injury. Due to the length of time that has elapsed, staff had no recollection of the specific incident and there was no record in the C1’s file of an incident or injury for the specified date. LPA was not authorized to interview C1.

It was also alleged that on multiple unspecified dates prior to September 2024, facility staff did not release C1 to authorized representative. Facility staff denied the allegation explaining that they did not deny C1’s release, but were restricted by a court order and C1’s mother’s instructions not to release C1 to the father until a specific time of day. During interview with LPA, C1’s mother confirmed that the pickup parameters were outlined and agreed upon during a custody hearing conducted on October 13, 2022. Based on a review of the court documents, LPA was unable to determine if the time restriction applied to certain days of the week and/or whether or not, if C1 was at the facility or under the mother’s custody.

Due to the amount of time that has lapsed since the alleged incidents and a lack of supporting evidence and conflicting witness testimony, LPA was unable to determine whether or not the above allegations occurred. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

Exit interview was conducted and this report was reviewed with facility owner Lynn Twork. A Notice of Site Visit was provided and must remain posted for 30 days. LPA observed that the notice was posted during the inspection. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
