25 | On August 20, 2026, at 8:30 a.m., Licensing Program Analyst (LPA) Victoria Felix conducted an unannounced Case Management inspection regarding a self-reported incident. LPA met with Director Susanna Herbst and disclosed the purpose of the inspection. LPA, accompanied by the Director, toured the indoor and outdoor areas of the facility to gather census. There were 48 children present, supervised by two 13 staff members.
Director reported that on 8/12/2026, Staff #1 (S1) and Staff #2 (S2) were preparing to take eleven children to the playground located behind the secured building. Prior to leaving the classroom, S1 and S2 did not complete the required name-to-face verification. As a result, Child #1 (C1) was inadvertently left inside the RM #3 classroom from approximately 4:00 p.m. to 4:04 p.m. An incident involving a lack of supervision of C1 was subsequently reported. At approximately 4:04 p.m., a parent discovered C1 inside the classroom and notified the Director.
Interviews were conducted with the Director, and S2. The LPA did not interview C1 due to the child's nonverbal status. The LPA reviewed staff files and obtained copies of the facility roster, sign-in/sign-out sheets, and other relevant documentation.
Based on the interviews conducted and documentation reviewed, it was determined that C1 was left alone and without supervision inside the classroom located within the secured building for approximately four (4) minutes. S2 stated that C1's authorized representative was notified of the incident.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, a deficiency is being cited on the attached LIC 809D. This violation constitutes an immediate civil penalty for Absence of Supervision, Section 101229(a)(1). A $500 immediate civil penalty was assessed. Refer to LIC 421IM.
