25 | On 02/01/2022 at 11:25 AM, Licensing Program Analysts (LPAs), Michelle Hood and Cindy Meier arrived to conduct an unannounced Plan of Correction (POC) inspection to follow up on deficiency cited on 01/19/2022. Upon arrival, LPAs met with the Facility Director, Christina Martinez (Tina) and toured the facility.
LPAs observed:
Four (4) children with one (1) staff in the Zebra classroom
Twelve (12) children with three (3) staff in the Seals classroom,
Fifteen (15) children with three (3) staff in the Chipmunks classroom
Twelve (12) children with three (3) staff in the Raccoons
LPAs observed four children and one staff with face coverings transitioning from the playground to the Zebra classroom. There were six children and one staff in the Seals classroom wearing face coverings. There were six additional children and two staff with face coverings transitioning from the playground to the Seals classroom. There were 15 children with three staff transitioning from the playground into in the Chipmunks classroom with face coverings. There were 12 children eating lunch with three staff wearing face coverings in the Raccoons classroom. LPAs observed the children's face coverings stored individually in a plastic labeled baggie within child's reach. LPAs conducted follow-up interviews with facility staff and confirmed facility COVID policy has been updated.
During the inspection, the deficiency cited on 01/19/2022 was cleared and the POC clearance letter was provided to the Facility Director, Tina Martinez. The Facility Director, Tina Martinez was provided the appeal rights (LIC9058 01/16) and her signature on this form acknowledges receipt of these rights. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and the report was reviewed with the Facility Director, Tina Martinez. |
