On 12/27/21, at 9:28 a.m., Licensing Program Analyst (LPA), Rajani Goudreau, conducted an unannounced Annual Inspection and met with Director, Christina Martinez. LPA disclosed the purpose of the inspection and toured the facility indoors and outdoors. This is a full day program which operates year around, Monday through Friday from 8:00 a.m. to 5:30 p.m. There are currently four classrooms in operation (#128, #130, #134, #138). The following ratios were observed: 20 children with nine staff members.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection. All poisons shall be kept in a locked storage area. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off-site activities. See LIC809-C continuation page...

The name of the childcare center director or fully-qualified teacher has been designated to act in the director’s absence has been reported to the Department.The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children’s files and observed contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment on file. However, two children (children #1 & #2 -listed under the LIC811) ages 18 through 24 months of age enrolled in the toddler component did not have documentation on file reflecting the sleep checks, based on review of files. LPA informed the director the provisions of sections 101351 through 101439.1 shall apply for children between the ages of 18 and 24 enrolled under the toddler component. LPA reviewed a sample of staff files and observed the health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training on file. However, staff #1 listed on the LIC811 did not have at least six post-secondary semester or equivalent quarter units of the required education or a child development assistant or teacher credentialing permit, based in review of the transcripts on file. Teacher qualifications were discussed with the director. The hiring is conducted by the licensee. Menus are posted at least one week in advance where an authorized representative can view them.

A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. This facility provides Incidental Medical Services (IMS). There is currently no children in care receiving IMS services. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following two type B deficiencies are being cited: (see the LIC809-D page). Exit interview was conducted and reports were reviewed with Director, Christina Martinez. A notice of site visit was given and must remain posted for 30 days.
