On 3/28/2022, at 11:00am, Licensing Program Analyst (LPA) Martha Malane, conducted an unannounced Annual Inspection and met with Director, Perla Zazueta. LPA disclosed the purpose of the inspection and toured the facility indoors and outdoors. This is a full day program which operates year around. Days and hours of operation are Monday – Friday 6:00am – 6:00pm. There is currently one (1) classroom, room #2 in operation. There are no children enrolled at this time.

Director stated there are no bodies of water on the premises and LPA did not observe any bodies of water on the premises. Director stated there are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions and other hazardous items are made inaccessible. No poisons were observed during the inspection.

Furniture and equipment are in good condition at the time of the inspection. Playground equipment is in safe condition at the time of the inspection. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. Solid waste storage containers have tight-fitting covers and are in good repair. Facility has a functioning carbon monoxide detector that met statutory requirements.
A review of staff records on this date indicates facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA reviewed staff file and observed file was complete with current CPR/First Aid certificate health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training.

Incidental Medical Services (IMS) was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. No deficiencies cited during today's inspection.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
