On 03/11/2024 at 11:30am, Licensing Program Analyst (LPA), Samantha Clenista conducted an unannounced case management site inspection to address facility’s change of capacity application. Upon arrival, LPA met with Facility Representatives, Josefina Curiel (Center Director) and Lisa Grygera (Area Director). All indoor and outdoor activity space utilized for the children were inspected today. The facility is requesting to be licensed for 18 infants (6 weeks-24 months) in Classrooms #1, #2 and #5 and 24 toddlers (18-36 months) in Classrooms 3A and 3B, total capacity shall not exceed 42 children. Fire clearance was granted on 01/30/24. The indoor areas used for the infant and toddler program were observed to be separate from one another and from the preschool program. LPA informed Facility Representatives that the programs shall never commingle at any given time and that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor play. LPA observed a total of 15 children with 8 staff members.

Previous measurements of Classrooms #1 and #2 were taken by LPA Rajani Goudreau on 05/05/2021. Facility Representatives stated there were no changes made within these classrooms since it was first licensed. The only change is that the facility added Classroom #5 to be an infant room. LPA observed that the sketch was slightly tweaked from the first initial sketch to the most updated for Rooms #3A and #3B. LPA inspected and measured Classrooms #5, #3A and #3B during inspection. The total indoor activity space for Classrooms 1, 2, 3A, 3B and 5 measured at approximately 3,680.01 sq. ft., sufficient for requested capacity. The infant and toddler rooms currently have a changing table within arm’s reach of a sink. There are 2 toilets and 3 sinks available for the toddler children. There is a refrigerator for children’s use in all classrooms. LPA reminded Facility Representatives that the toddler program shall have two separate groups of 12 and groups shall not be combined for activities exceeding 12 children in each group. Classroom #3A and #3B are separated using a room divider.

There are only two separate crib areas located in Infant Classroom #1 which currently has 6 cribs in total. LPA observed that the crib area was not fully enclosed and that some of the walls barricading the crib area were measured at less than 4ft. high. There are napping cots in the other classrooms used for older infants/toddlers. Facility Representatives were reminded that all infant bottles shall have their name and date on them daily. Infant needs and services plan are stored in the classroom in a binder per child and Facility Representatives were reminded that they have to be updated quarterly. LPA observed all indoor play space to be complete with age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for the children. There is no change in staff restroom, isolation area and bathroom. There is an operational carbon monoxide detector on site located in each classroom. All required documents were observed posted in the main parent drop off/pick up area.

The facility currently has an approved playground waiver that allows them to share their outdoor playground area between their infant and toddler children. LPA remeasured all areas since there was change of outdoor patios. The shared playground located in the middle of the lobby and Toddler Classrooms measured at 996 sq. ft., sufficient for only 13 children at one time. The facility has an enclosed outdoor patio that are attached Classroom #1, #2, #3B and #5. The one attached to Classroom #3B is designated for their toddler children. LPA measured all outdoor patios. The Facility Representatives requested that the shared playground still be shared between their infant and toddler children, however the outdoor patios that are attached to the above-mentioned classrooms, can be used solely for the children in each classroom. Facility will need to submit and obtain approval of an updated playground waiver to share the one playground between infants and toddlers at different times in the AM and PM and that no more than 13 children will play out at one time. Due to limited square footage for their infant and toddler outdoor play space, another playground waiver shall be submitted and approved prior to licensure. Areas were observed to be fully fenced. Area had a pop-up canopy used for shade. Area had age-appropriate toys and play equipment and had rubber turfing under play structure, sufficient to sustain a fall. Facility Representatives were reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

Prior to granting facility’s capacity change, the following is needed:
  • Submit a playground waiver request to share the middle playground between their infant and toddler children at separate times in the AM and PM (see written directives from PIN)
  • Submit a separate playground waiver request to allow sharing of the total infant outdoor space and total toddler outdoor space due to limited square footage (see regulation 101238.2(a)).
  • Submit proof of installing either a gate or fence to enclose both crib areas located in Infant Classroom #1.
  • Submit proof of extending the barrier of both crib areas located in Infant Classroom #1 to meet 4ft. or higher made up of sound absorbing material.

Exit interview was conducted and report was reviewed with Facility Representatives. A license for 18 infants (6 weeks-24 months) in Classroom #1, #2 and #5 and 24 toddlers (18-36 months) in Classroom 3A and 3B may be granted upon receiving the above corrections and after conducting a final file review. Facility Representatives stated their understanding. Facility Representatives understands that this report shall be kept available at the facility for public review for 3 years.

Due to computer malfunction, LPA will email a copy of this report along with the Notice of Site Visit and appeal rights to Director by 5pm today. Director is advised that her response to the email within 24 hours of receiving it will act as her signature. Director is aware that the Notice of Site Visit must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
