25 | On 08/22/2023 at 10:50 am, Licensing Program Analyst (LPA) Michelle Hood, made an unannounced inspection to follow up on a self reported incident that occurred on 08/01/2023, wherein a 2 year old child was grabbed by a teacher while in care. The director lead the LPA on a tour of the facility. There were nine children with three teachers present at the time of the inspection.
LPA reviewed children and teacher files. LPA interviewed the director, one teacher, and two children. There was no video surveillance to review. As of 08/02/2023, the teacher was terminated from the facility. There was one teacher and one teacher's aide with approximately 6 to 7 children on the day the incident occurred. Further follow up is needed. LPA will complete the follow up and make a determination whether there has been a regulatory violation.
An exit interview was conducted and the report was reviewed with the director Tiffany Burroughs. The licensing report will be e-mailed to the director and the director was advised that acknowledgment of receipt of the report and appeal rights (LIC 9058 12/15) are to be received within twenty-four hours. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. No deficiencies were cited during today's inspection. |
