On 10/11/21 at 12:01 p.m., Licensing Program Analysts (LPAs), Rajani Goudreau and David Miller arrived at the facility to conduct a pre-licensing inspection. Upon arrival LPAs, met with the President, Jennifer House, and VP of Operations, Jennifer Grawvunder, and proceeded to tour the facility. The facility is requesting a capacity of 34 school-age children; ages 5 through 16 years old in room #14. Facility plans to operate Monday through Friday from 6:00 a.m. to 6:00 p.m.
The classroom was inspected and measured during this inspection and measured the following:
- School -Age Monsters Classroom - #14: School Age (serves children 5 through 16 years of age): Measured 1,199.51 square feet.
The classroom measured a total of 1,199.51 square feet which is sufficient to accommodate 34 children. Applicant representative agrees to lower the capacity to 34 children due to insufficient indoor space, updated LIC200A (Application) received during visit. The classroom is equipped with appropriate furniture; including tables, chairs, cubbies, bookshelves, and other materials for the use of the children. LPA observed the furniture and equipment in good condition, free of sharp, loose, or pointed parts. There is a functioning carbon monoxide detector and a first aid kit with required supplies located in the facility. Disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible and poisons shall be locked. Drinking water is available in the classroom via refillable water containers and cups provided by the facility. Parents will be utilizing digital sign in and sign out. There is three restrooms, each restroom containing one sink and one toilet for the children's use, offering individual privacy. In addition, there is one sink located in the hallway of the classroom. Therefore, there is four sinks and three toilets available for the children's use. There is sufficient toilets and sinks to accommodate the requested capacity. See LIC809-C continuation page... |
