On 7/26/2024 at 2:40 pm an office meeting was conducted with Director Xavier Deanfrasio, Regional Director Nichelle Gissler and Facility Representative Michael Ong. Director and Regional Director attended the meeting in-person and Licensee Representative attended via zoom. Also present were Licensing Program Managers (LPMs) Cynthia Gray and Tulam Vu and Licensing Program Analysts (LPAs) Adrian Castellon and Julieta Abrego. The facility citation and compliance history and the assessment of civil penalty for absence of supervision during an inspection dated 5/10/24 were discussed.

On 5/10/24, the facility was issued a Responsibility for Providing Care and Supervision Type A citation. On 4/15/24, the facility was issued three Type B citations. On 1/5/24, the facility was issued a Responsibility for Providing Care and Supervision Type A citation. On 11/1/22, the facility was issued a Personal Rights Type A citation. The facility was previously placed on a Non Compliance Plan from 2/7/23 through 2/7/25.

The facility has received three (3) Type A citations and four (4) Type B citations since being licensed in January 2022, including the following citations: personal rights and responsibility for providing care and supervision.

LPMs and LPAs discussed the above citations with Licensee Representative, Regional Director and Director. Licensee Representative, Regional Director and Director were provided the following regulations: Sections 101214 Accountability, 101216 Personnel Requirements, 101215.1 Child Care Center Qualifications and Duties, 101415 Infant Care Center Qualifications and Duties, 101429 Responsibility for Providing Care and Supervision, and 101223 Personal Rights.

Licensee Representative, Regional Director and Director were informed that repeated violations or failure to comply with licensing laws and regulations may result in administrative action taken by the Department. Licensee Representative, Regional Director and Director stated they understood and will ensure the facility complies with all regulations and laws governing Child Care Centers.

Regional Director and Director stated that a revised plan of correction will be submitted to the Regional Office by August 9th, 2024. The plan of correction will include the following: updated LIC500; updated LIC309; detailed plan of how director will provide direct observation of the staff in the classroom; director will send a reminder to all parents of the importance of closing all gates and doors to ensure the health and safety of children in care; removal of cubby/shelf in front of infant class door. Licensee Representative stated they will look into IT options to alert staff of classroom door opening.

A civil penalty of $500 was issued on this date via LIC421IM due to computer technical issues. Please see file for signatures.

The Licensee Representative, Regional Director and Director were provided with the following resources: A Technical Support Program (TSP) brochure and the TSP email: childcaretechnicalsupport@dss.ca.gov. Licensee Representative, Regional Director and Director were advised to notify LPA if interested in participating in TSP. LPMs recommended that facility staff review the following videos from https://ccld.childcarevideos.org: Supervising Children in Child Care Center and Children's Personal Right in Child Care. Child Care Advocate information was provided: (714) 703-2800 or childcareadvocatesprogram@dss.ca.gov.

An exit interview was conducted with director and report was reviewed. A copy of appeal rights (LIC9058) was provided and their signature on this form acknowledges receipt of these rights.
