On 11/1/22 at 1:45pm, Licensing Program Analyst (LPA), Martha Malane conducted an unannounced case management inspection to follow up on three (3) separate self-reported incidents to the San Diego Regional Office. Upon arrival, LPA met with Director, Xavier Deanfrasio and disclosed the purpose of the inspection. LPA toured the facility. There were 19 infant children and five (5) staff members present.
Two (2) separate unusual incident reports (UIRs) were submitted to the San Diego Regional Office (SDRO) on 10/12/22. UIRs were submitted alleging a possible personal rights violation and lack of supervision which occurred on or about 10/11/22. A third UIR was submitted on 10/21/22 alleging a possible personal rights violation on or about 10/11/22.
The first UIR alleging a possible personal rights violation was reviewed. Based on interviews with staff and parents and video footage reviewed, Staff 1 (S1) was observed yelling in the face of and handling Child 1 (C1) in a rough manner. On or about 10/11/22, S1 was observed grabbing the arm of C1, picking C1 up and roughly placing C1 on the floor. Based on information obtained, S1 violated the personal rights of C1.
The second UIR alleging lack of supervision was reviewed. Based on interviews with staff and video footage reviewed it was determined that on 10/11/22, Staff 2 (S2) was observed leaving classroom 4 at 12:55pm and left four (4) sleeping infants unsupervised. Based on video footage reviewed at 12:55:36pm the director was observed entering the classroom to supervise the infants. Based on information obtained, infants were left unsupervised for about 36 seconds.
The third UIR alleging a possible personal rights violation was reviewed. Based on interviews and video footage reviewed, S3 was observed grabbing C2 under the arm, picking C2 up and roughly placing C2 in a chair. Based on information obtained, S3 violated the personal rights of C2.
