On 5/11/23 at 9:25am, Licensing Program Analyst (LPA), Martha Malane conducted a Case Management Non-Compliance inspection regarding a Non-Compliance Plan dated 2/7/23. Upon arrival, LPA met with Director, Xavier Deanfrasio and discussed the purpose of the inspection and was led on a tour of the facility. There were 10 children and four (4) staff members present. Hours of operation are Monday – Friday 7:30am - 5:30pm.

LPA observed appropriate ratios as well as proper care and visual supervision of children during the inspection. LPA also confirmed that the facility is operating under the Prevention Plan submitted to the Department.

No deficiencies cited.

Exit interview conducted with Director, Xavier Deanfrasio. Notice of Site Visit was provided and shall be posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100.
