On 4/15/2024, at 9:55am, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced annual inspection. LPA met with Facility Representative, Xavier Deanfrasio and disclosed the purpose of the inspection. LPA inspected and toured the indoor and outdoor of the facility. The following ratios were observed: Room 1 had 4 infants in the classroom with two teachers and two infants from room 1 were observed on the playground with one teacher; Room 2 had four infants in the classroom with Assistant Director, Francisa Lumaban and one teacher, seven infants from room 2 were observed on the playground with two teachers and one assistant. Days and hours of operation are Monday through Friday from 7:30 am to 5:30 pm.

Furniture and equipment are in good condition. Floors in the facility are clean and safe. Disinfectants, cleaning solutions, medications and other hazardous items are made inaccessible to children. Potty chairs, toilets and hand-washing equipment are in safe and sanitary operating condition. Kitchen, food preparation and storage areas are clean, free of litter/rubbish. Solid waste storage containers have tight fitting covers and are in good repair. Playground equipment is in safe condition. The surface of the outdoor activity space is maintained in a safe condition and free of hazards. Facility has a smoke and carbon monoxide detector that meet statutory requirements. The last fire/disaster was conducted and documented on 1/31/2024.

Facility Representative stated that there are no swimming pools or other bodies of water on the premises. Facility Representative stated that there are no firearms or ammunition allowed or stored on the premises.

The facility provides breakfast, lunch and snacks to children starting at one years of age and up to two years of age. Children bring their own water bottles and filtered drinking water is available at the facility. Formula is provided by the parents along with bottles. LPA observed that bottles for Child 1 (C1) and Child 5 (C5) located in the refrigerator in infant room 1 did not include the date. See LIC 809C Continuation...

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than four (4) children in care. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative’s absence has been reported to the Department. Facility utilizes an electronic system for sign in and sign out procedures. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment; however Infant Needs and Service Plans were not updated for Child 1 (C1), Child 2 (C2), Child 3 (C3) and Child 4 (C4). LPA observed that 15 minute sleep checks between 8:45am and 10:05am were not documented for C1. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA observed Notice of Site Visit posted on the bulletin board in the lobby.
