On 08/09/2024 at 11:25 am, Licensing Program Analyst (LPA), Dana Stevens conducted an unannounced case management inspection for the purpose of a room addition. LPA met with Director Crystal Kremensky and proceeded to tour the facility. Licensee is requesting to add Room 2 to their existing license serving 15 preschool children age 2 years – 5 years. Facility hours of operation are 8:00 AM – 4:00 PM.

· Room 1: Previously measured 910.13 square feet, serving Preschool children
(ages 2 – 5 years)
· Room 2: Measured 649.49 square feet, serving Preschool children (ages 2 - 5 years)

Room 2 indoor space for children measured a total of 649.49 square feet which is sufficient to accommodate a capacity of 18 children, however licensee is requesting capacity of 15 at this time. Classrooms are equipped with age-appropriate furniture including mats for napping, tables, chairs, cubbies, bookshelves and other materials for children. There are two (2) toilets and three (3) sinks available for use, which is sufficient to accommodate the requested capacity.

Director stated drinking water will be available inside and outside via water fountains and children bring individual, refillable water bottles. There is a separate restroom for staff located in the adjacent cafeteria building. The school nurses office will be utilized for isolation if a child becomes ill, and there is one toilet and sink available for children's use. Disinfectants and cleaning solutions are kept in high cupboards but were not made inaccessible to children.

The facility provides snacks and meals delivered by the school cafeteria. The facility utilizes hard copy sign in sheets. There was no operational carbon monoxide detector. There is a first aid kit available with required supplies. Required documents were posted.
A fire clearance for 15 preschool children was granted 01/24/2024.

The outdoor activity space was inspected and previously measured a total of 4,179 square feet which is sufficient to accommodate 55 children. The outdoor space is located on the east side of the campus. Children shall be escorted to and from the outdoor activity space by a teacher. Shade is provided through the use of trees. The preschool outdoor activity space is for exclusive use by preschool children.

Incidental Medical Services (IMS) was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following corrections are needed for approval of Room 2:
  • Operational Carbon Monoxide Detector
  • Disinfectants and Cleaning solutions made inaccessible to children

An exit interview was conducted with Director and a copy of this report was provided. Notice of site visit must be posted for 30 days.
