32 | At 3:07pm LPAs observed Child 1 (C1), who is 6 years old, and Child 2 (C2), who is 15 years in room 2 with preschool children. The facility is currently licensed for preschool children age 2-5 years; see LIC809D for deficiency cited as licensee is operating beyond the conditions and limitations specified on the license.
LPA informed Licensee, Laura Valdez-Oropeza that this report dated 7/26/23 documents one (1) Type A citation on the attached LIC809-D, which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed licensee to provide a copy of this licensing report dated 7/26/23 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child's file for verification.
The following is required:
1. Plan of operation for toddlers and preschool children to share the restroom between rooms 3 and 4
2. Updated LIC200A to reflect a decrease in the requested capacity
3. Proof of room 4 set up for children
Exit interview conducted with Licensee, Laura Valdez-Oropeza. Notice of Site Visit shall be posted for 30 days from today's date. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
A license for 60 preschool children (age 2-5 years) in rooms 1, 2, 3 and 5 and 11 toddler children (age 18-36 months) in room 4 may be granted after required documents are submitted and approved and a final file review. |
