On 9/14/22 at 9:30am, Licensing Program Manager (LPM), Tulam Vu and Licensing Program Analyst (LPA), Martha Malane arrived at the facility to conduct a pre-licensing inspection. LPA met with Senior Director, Beverly Jensen and Director Tina Smith and was led on a tour the facility. Applicant is requesting a capacity of 95 children (age 2-5 years). Applicant’s planned hours of operation are Monday - Friday 7:00am – 5:30pm.

The indoor space measured a total of 3248.48 square feet which is sufficient to accommodate 92 children. Applicant provided an updated LIC200A to reflect a requested capacity of 92. Classrooms are equipped with age appropriate furniture and equipment including cots for napping, tables, chairs, cubbies, bookshelves and other materials for children. There are nine (9) toilets and 11 sinks available for use, which is sufficient to accommodate the requested capacity. Drinking water is available outdoors and indoors via water pitchers and refillable water bottles brought from home. There is a separate restroom for staff located upstairs next to the office. One of the toilets and sink in the Early Learners restroom will be utilized as an isolation toilet for children who are ill. The isolation toilet and sink shall only be utilized for children who are ill and will marked with a sign to indicate the use. The isolation area in the Early Learners 1 class will be utilized if a child becomes ill. Disinfectants and cleaning solutions are kept in an inaccessible cabinet in the kitchen. The facility provides meals and snacks via a catering company. The kitchen includes one (1) sink, one (1) refrigerator with a freezer, one (1) stove and storage space. The facility utilizes manual sign in and sign out sheets. Required documents are posted. There is an operational carbon monoxide detector.

The outdoor activity space for preschool children was inspected and measured a total of 13,134.35 square feet which is sufficient to accommodate 175 children. The outdoor space is fenced and shade is provided by the use of shade sails and trees. Proper supervision and placement of staff outside was discussed to ensure the safety of children at all times. Director stated the three tiered outdoor Nature Lab is only utilized by the 4 – 5-year-old children and there are no more than five (5) children in the Nature Lab at one time. LPA and director discussed supervision of children while utilizing the Nature Lab.

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Director stated there are no bodies of water on the premises, Director stated there are no weapons, firearms or ammunition on the premises,

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA and director discussed water testing in child care centers and LPA provided PIN21-21CCP.

A Fire clearance for 95 children (age 2 – 6 years) was granted 5/31/22.

Exit interview was conducted with Director, Beverly Jensen. A license for 92 preschool children (age 2-5 years) may be granted after a final file review.
