On 09/23/22 at 9:21 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a pre-licensing inspection. Upon arrival LPA, met with Administrator’s and Owner’s, Gilberto Hernandez, Jimena Barrull and Director, Corina Tellez and proceeded to tour the facility. The facility is requested a capacity of 30 children; serving ages 2 years old through Kindergarten in classrooms A and B. Facility plans to operate: Monday through Friday from 8:00 a.m. to 3:45 p.m.

The classrooms were inspected and measured during this inspection and measured a total of 1,486.44 square feet which is sufficient to accommodate 42 children at one time.The classrooms are equipped with age appropriate furniture and napping equipment (cots) including tables, chairs, cubbies, bookshelves, and other materials for the children. Classroom A contains one portable sink for the children’s use. Also, there is one portable sink located in Patio #1. Plan of operation on file and approved for the use of the portable sinks. There is one restroom near patio #1 containing one sink, two toilets and one urinal. There is insufficient toilets to accommodate the initial requested capacity of 36 children. Facility representative has agreed to lower the requested capacity to 30 children. Updated LIC200A received at time of inspection. Children are to be escorted, supervised with direct supervision by staff to and from the restroom and during outdoor activities. Drinking water is available in the classrooms via a filtered water dispenser and during outdoor activities by the use of a refillable water container. Children will bring water bottles from home or the facility will provide water cups if needed. The staff lounge area and staff restroom will be used as an isolation area for ill children, if needed. There is a functioning carbon monoxide detector in the classroom and a first aid kit with required supplies, per observation. Disinfectants, cleaning solutions and other items that could pose a danger shall be stored where they are inaccessible to children. Breakfast, Lunch will be brought from home. Snack will be provided by the facility. Required postings are posted near the front entrance of classroom A, per observation. The facility will be utilizing manual sign in and sign out. There are no bodies of water on the premises of the property, per observation and applicants statement. The outdoor activities spaces include patio #1, patio #2 and outdoor play area. Patios #1 and #2 are located behind classroom A. The other outdoor play area is located near the front of classroom A. The outdoor activity areas were inspected and measured a total of 2,979.85 square feet which is sufficient to accommodate 39 children. The surface of the outdoor activity spaces is maintained in a safe condition and free of hazards. Areas around climbing equipment was observed to have rubber matting material for cushioning to absorb falls. There is shade in the outdoor activity spaces by the use of shade kites, trees, a building awning and a movable canopy. The play equipment is suitable for ages 2 through 5 years old. Proper supervision and placement of staff during outdoor activities was advised to ensure the supervision and the safety of children.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with applicant, the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

This facility provides Incidental Medical Services - IMS. IMS plan submitted and approved. Initial water lead testing results shall be received and posted within 180 days of licensure. LPA provided and discussed the following: PIN 21-21-CCP, External Water Sampler Self-Certification Form (LIC 9275), Child Care Center Sampling Checklist Form (LIC 9276).

A fire clearance for 36 children; age 2 years old through Kindergarten in classrooms A and B was approved on 09/01/22. An updated fire clearance will be needed due to the change in the requested capacity from 36 to 30 children.



Exit interview was conducted with Administrator and Owner, Gilberto Hernandez. After final review, a license for 30 children; ages 2 years old through Kindergarten in classrooms A and B shall be granted. Once the license is issued the license will be mailed to the facility. Once received the license shall be posted in a prominent location in the facility.
